We are happy to welcome our own Care Partners staff who will tell us about the Advance Planning process, including Healthcare Directives and Health Care Agents. At this session, we’ll review questions to consider as we make our choices, provide a brief overview of common end-of-life medical terms, and help us know when to review or revise our advance planning documents.
